    Infection Prevention & Control (IPC) Supervisor

    Main objective and responsibility of the position

    • Implementing and supervising an infection control program in a project or health facility, according to MSF protocols, safety and IPC measures, in order to ensure quality of care and safety of patients, caretakers and staff

    Accountabilities

    • In collaboration with each service activity manager, maintaining and supporting the day to day implementation of the standard precautions in health structures in all departments including sterilization, laundry and kitchen, and providing technical supervision, training and support to the staff on IPC related issues using existing tools such as audits, checklist, surveillance
    • Implementing and monitoring adherence to additional precautions for air, droplet and contact transmissible diseases when indicated. Based on a given patient-diagnosis, evaluating the need and supporting the implementation, if needed, for additional precautions in collaboration with the medical team and act accordingly
    • Implementing as planned, the correct ‘clean and dirty flows’ inside the health facility. Ensuring that environmental standards in the facility are respected (bed spacing, number of handwashing stations etc.) and together with the Hospital Facilities Manager and WHS Supervisor, that the essential requirements are available in the health structures
    • Guaranteeing compliance to aseptic care techniques and to the correct use of antiseptics, disinfectants and detergents.
    • Checking the consumption and the quality of IPC materials and equipment, including cleaning materials and equipment, PPE, etc.
    • As member of the Infection Control Committee, participating in and supporting the development and following up the Infection Control Action Plan.
    • Supervising the team under his/her responsibility and participating in the associated HR processes (e.g. recruitment, training/induction, evaluation, potential detection, coaching, development and communication) in order to ensure both the sizing and the amount of knowledge required.
    • Reporting any irregularities to NAM or IPC managers or PMR and producing reports with information recommendations for the monthly medical reporting

    Note: IPC standard precautions include Personal Protective Equipment (PPE), hand hygiene, prevention of accidental exposure to body fluids, safe and appropriate treatment of re-usable medical devices, waste management, cleaning and disinfection of surface and environment, respiratory hygiene and individual hygiene for staff, caretakers and patients IPC essential requirements include water supply, solid waste and waste water treatment/disposal, latrines and shower facilities, dead bodies management, vector control and medical waste management

    MSF Section / Context-Specific Accountabilities

    • The IPC Supervisor in Zamfara Project is the main responsible for the implementation of all IPC related activities, protocols and different recommendations to ensure quality of care and patient safety in MSF Health Facilities including Anka General Hospital NEP IDP clinic and Talata Mafara ITFC ward . The IPC Supervisor works closely with all multidisciplinary team members to raise awareness and ensure IPC issues are identified and addressed by all staff members.
    • As a member of the Infection Control Committee, participating in and supporting the development and follow up of the Infection Control Action Plan and other communication activities are key (may support MTL with minutes/agenda/action points, also may chair the meeting in absence of MTL)

    Liaison with all Managers/Supervisors and Balanced Leadership and Communication Skills:

    • In collaboration with WatSan team, ensure maintenance of a clean and hygienic environment.
    • In conjunction with Supervisors/Managers, carry out training of all staff (Medical, Lab, Hygiene Agents, Watchmen etc.) on relevant IPC information - IPC Pillars and aseptic technique.
    • In collaboration with ITFC Matron in Talata Mafara ensure IPC protocal and policy are in place in the ward, milk kitchen and in the waste zone.
    • In Conjunction with WatSan focal person and Nursing Team Supervisor ensure waste segregation at the source and subsequent collection in different wards and keep high hygiene standard.

    Stock Management:

    • Under leadership of Nursing Activity Manager and Pharmacy Supervisor ensure ordering, storage and management of IPC materials (hand sanitizers, surfanios and hexanios, chloramine solutions, face masks, disposable gowns etc) for daily usage.

    Supervision & Policy /Protocol Implementation:

    • Supervise, coach and support hygiene agents working in the Hospital (APW) and IDP through duty allocation, making roster, leave plan and perform yearly performance evaluation.
    • Provide technical support to the Laundry team by ensuring clean and adequate bed linen are in circulation.
    • Ensure vector control in the hospital wards and other departments(use of light insecticide etc).
    • Supervise and support IRS and deep cleaning in the various wards.
    • Reinforce the implementation of the standard precaution in all departments ie. Use of PPE, hand gloves, prevention of needle stick injuries.
    • Ensure safe and appropriate treatment of re-usable medical devices(cleaning ,basic disinfection,high disinfection or sterilization.
    • Organise and lead regular IPC meetings in the hospital and report on the results, challenges and action plan.
    • Policy management of IPC activities in all locations and work in conjunction with all heads of departments to ensure and supervise implementation of IPC policy and protocols.
    • Supervises directly the Sterilization Technician and ensure small surgical materials are safe for patients, sterilization procedures follow IPC protocols, and provide necessary training.
    • Ensure compliance of the 3 IPC pillars (Hand hygiene, Cleaning and Disinfection, transmission based precaution) in MSF health facilities including Anka General Hospital and IDP Clinic.

    Reporting:

    • Be responsible for monitoring of hand hygiene compliance using the hand hygiene observation tool (HHOT) and monthly updating of results and communicate the score to the team.
    • Carry out SIPCA assessment with support from line manager and subsequent writing of action plan after the SIPCA score.
    • Report IPC activities in the MMR
    • MSFAT assessment.

    Other:

    • Gap fill WatSan focal person in case of absence.

    Medical Doctor

    Main objective and responsibility of the position

    • Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions.

    Accountabilities

    • Apply medical knowledge and skills to diagnose and prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
    • Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
    • Follow up the evolution of the hospitalized/IDP patients, through daily visits , consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution.
    • Check and control the rational distribution of medicines and equipment under his/her responsibility and take care the quality, disinfection and sterilization of the medical material. Assure general compliance with standing hygiene standards.
    • Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patients’ illness, medical error and monitors the proper functioning of the department, equipment or material.
    • Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care.
    • Knows and ensures all MSF medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control. Ensures professional confidentiality is respected.
    • Manage the team under his/her direct responsibility according to MSF HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.

    MSF Section / Context-Specific Accountabilities

    • The Medical Doctor under MTL leadership is overall responsible for medical care provided to all patients accessing the pediatric ward of Anka General Hospital from the screening tent to the ward through triage and Emergency room and OPD locations(IDP Camp and ATFC). He is leading patient care in close collaboration with Nurses , Clinical Assistants, Hygiene agents, lab technicians etc to ensure patient safety.

    General:

    • Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.

    Patient care Management:

    • Knows, understands and complies with admission and discharge criteria available in the ER and strict adherence to it.
    • Strict adherence to the admission process, this includes the correct use of the admission file(pink for non-SAM and Yellow for SAM patients) respecting all steps of patient examination and initial Management.
    • Writes clearly complains and take meaningful history of the current sickness, Physical examination, Lab investigation, initial diagnosis and treatment plan.
    • Apply actively ETAT principle to identify and treat urgent patients at higher risk and ask for support whenever needed.
    • Carry out daily ward round in the assigned ward, update diagnosis according to new findings and adapt the treatment plan. Deep and detailed patient examination after thorough review of documentation from the previous colleague.
    • Discharge patients who meet the discharge criteria
    • Proactively Identify patients who need referral to another referral hospital, prepare the referral sheet, inform the caretaker then your line manager to organize the movement.

    Drug Management:

    • Correct use of the prescription for all oral drugs at discharge including bednet for all new admission
    • Participate in the Monthly consumption Analysis
    • Rational use of Antibiotics

    Data Collection and Reporting:

    • Fills the epi front page for all exits
    • Fills the inpatient Mortality sheet immediately after patient death.
    • Actively prepare and participate in the Mortality review case.
    • Writes Medical Incident Report whenever it occurs and report to the linemanager

    Medical Leadership:

    • The Medical Doctor is leading the overall patient care ,he should therefore be a very good communicator and a team player, guided by Medical Ethic, MSF principle and Values , respectful and respecting all (Para)Medical colleagues. Lead by example , with respect , integrity, humanity ,accountability and empowerment.
    • Efficient and professional communication with Nurses, Clinical Assistants, Nutrition Assistant and Hygiene Agent
    • Efficient and respectful communication with patients and their caretakers
    • Fully responsible of the patient care and outcome.

    Nursing Team Supervisor, ITFC

    Main objective and responsibility of the position

    • Plan, organize, and evaluate the activities concerning his/her field of action (INPATIENT THERAPEUTIC CARE) and the team associated, according to MSF values, policies and protocols and universal health standards, in order to warrant the quality and continuity of the health care and the development of the plan of action.

    Accountabilities

    • Carry out the functions and tasks associated to his/her speciality, i.e. perform as a nurse whenever required or needed, in order to optimize the resources, contributing with his/her knowledge and experience.
    • Implement all the protocols and hygiene procedures, and supervise his/her team follow the same standards, in order to warrant the quality of the care and service in his/her speciality.
    • Organize and coordinate the activities of his/her team (week’s schedule, annual leave, absences, etc.), evaluate their performance, define and ensure the needs for training of staff, in order to ensure the coverage of the human resources needs and maintain high standards of quality.
    • Carry out and/or supervise the (decentralized) pharmacy and medical equipment management (drugs orders, follow-up of the stock, storage conditions, inventories, drugs consumption, etc.) in his or her department, in order to satisfy the needs of material with efficiency and effectiveness.
    • Ensuring that all staff using medical devices are qualified and trained. Ensuring that cleaning and minor maintenance tasks are performed according to the protocols and listed items with any malfunctions are reported to the project biomedical service.
    • Carry-out and/or coordinate administrative (exit-paper, transfer-paper, etc.), information and data collection (patient files, forms, statistics, etc.) tasks, and elaborate regular reporting, in order to have updated and reliable information about the day-to-day activity in the project, output/ outcome and support decision-taking.
    • Give feedback/ reports to medical focal point.

    MSF Section / Context-Specific Accountabilities

    • Under the ITFC NAM Leadership, the ITFC Nursing Team supervisor is directly supervising and overseeing all the ITFC Nursing Activities, this includes Following up on ITFC patients admitted in isolation and general paediatrics HDU.
    • The NTS does not directly have financial responsibilities, all finance approval and HR needs (transport of Medical item to remote sites, Daily workers needs and their payment or any other payment process), all these are NAM/ITFC responsibilities.

    General:

    • Under NAM/ITFC leadership and in close cooperation, NTS is responsible for the day-to-day activities of ITFC department in Talata Mafara general paediatric wards of Talata Mafara general hospital.
    • Under leadership of the line manager and in close collaboration, the nursing team supervisor is responsible for the quality nursing care through mentorship, on Job supervision/trainings.
    • Prepares, provides and ensure all necessary stationaries and all related paper work are available in the inpatient departments (Registers, nursing notes, prescription, lab request forms…)
    • Together with Watsan and IPC supervisor ensures IPC measures fully implemented in all departments (ITFC wards: Phase 1&2): Cleanliness, beds making, no mats on the floor during the day, dirty linen stored in designated buckets covered with the lid, hand washing points, hand sanitizers…
    • Together with the HP Team ensure health message delivered on the waiting areas and addressing Health issues in line with main morbidities in the Health Facility.
    • Ensures good patient flow to allow reducing waiting time and unnecessary steps, the NTS should, Under the NAM leadership organize the departments to maximize efficient use of resources.
    • The NTS is technically supporting the team on the ground while major administrative tasks (daily worker request and signature on payment sheet for all incentive staff …) go to the NAM who is the main manager.

    Nursing Care:

    • Implement all MSF protocols, guidelines and Standard Operating Procedures (SOPs) that guide the day-to-day activities…
    • Supervise provision of quality care to the Patients (reception, admissions, patient care, safety and comfort, consultation, individual health education and drug provision) are timely administered, to avoid delays.
    • Ensure strict adherence and full implementation of applicable protocols on: monitoring of infusion, Medical (systematic treatment), Nutritional (following diets), Anthropometric (measuring weight/height etc.).
    • Support the shift-in charges to ensure preparedness through availability of all emergency stock, confirmed by updated checklists in all wards.
    • Ensure quality (aseptic technique) dressing following the wound care protocol.
    • Ensure complementary requests by the doctor, correctly applied in the desired period.
    • Put in place and maintain hygiene rules and universal precautions to staff involved inpatient care.
    • Ensure Medical staff under him/her know the PEP protocol (what to do in case of accidental exposure).
    • Ensure accurate documentation, reporting accurately done on patient’s folders, stock and all inventory registers.
    • Ensure the handing over report effectively carried out, with all duty staff informed of up-to-date changes on patient management.
    • Collaborate with MOH vaccinator, and ensure provision of vaccination for routine immunization to all children exiting ITFC department.
    • Ensure all staff are dressed in their prescribed uniforms while on duty.

    Data Management and Reporting:

    • Implement all data collection paper work in collaboration with the Epid- Department and the Nursing Activity Manager.
    • Checking that data collection forms are available and filled correctly (registers, forms, tally sheets, referral sheets and morbidity forms, etc.)
    • Compiling daily, weekly and monthly statistics for the ITFC activities and submit to epid-department and ITFC/NAM-OR respectively, as at when expected.
    • Draft the Monthly Medical Report for the activities she/he is supervising and submit to the line manager.
    • Give feedback to the nursing team, regarding progress in line with project-desired indicators, and discuss other possible measures for improvement.

    Pharmacy Management / Accountability:

    • The NTS is overall responsible for the drugs and Medical items stored in the Inpatient department, therefor she/he should ensure there is enough stock to run the wards, ensure good consumption tracking and submission to the central pharmacy with due respect to timing and ordering procedures.
    • Leads and deeply involved in the weekly inventories, double check consumptions and justify discrepancies.
    • Adheres to the project supply system and reinforce its implementation; Prepare Medical orders for the activity according to stock and consumption, submit to the line manager for further process.
    • Analyse consumption pattern and link them with morbidities, gives feedback to the team and provide necessary information to the pharmacy supervisor in order to adjust quantity of medical item to the need.
    • Check emergency boxes in the wards and ensure items used timely replaced.
    • Ensure rational use of drugs and strict adherence to protocols.
    • Ensure daily consumption is recorded, prescription well filled, stored and taken to the Hospital pharmacy.
    • Checks medical prescriptions carried out (treatment administration, surveillance of medical parameters, etc.)
    • Supervises, in cooperation with the pharmacy supervisor , the management of the pharmacy, ensuring reception and storage of orders, preparing new orders when required, supervising expiration terms and consumption patterns, in order to ensure, a rationale use as well as pharmacy stock levels are permanently updated and above minimum safety point etc.

    HR Management and development:

    • As a direct supervisor of the nursing team , the NTS has the duty to Know job descriptions of all staff working in the Inpatient department (nurses, nutrition assist, cooks, etc.) and checking that each individual knows and understands his/her own job and work accordingly.
    • The nursing team supervisor can identify the need of daily workers; follows on daily basis the work of incentivized staff and report to the NAM/ITFC the main responsible.
    • Leads staff under his/her supervision toward MSF objectives, promote medical ethics and patient safety.
    • In collaboration with Ward Matron draft Rosters timely of all personnel under his/her supervision and submitting to his/her line manager for perusal.
    • Identify training needs and organizes sessions within his/her team or within the general framework of personnel development.
    • In collaboration with the ward matron draft objectives of each staff member under her/his supervision.
    • Participates in the orientation of new personnel.
    • Organizes regular team meetings to encourage the exchange of information and maintain efficient communication within the team.
    • Keeps the team dynamic and promote good working atmosphere.
    • Reports to the line manager (NAM) any work related problem/ incident identified within the team.
    • Organize and conduct regular in-service/on the job training for the nursing staff and CHEWS.
    • NTS should attend to patient when the occasion arises as well as covering staff absence when decided by the line manager etc.

    Others:

    • Checking that all information is passed on during transfers/referral to the ITFC/IPD or other health structures in -order to ensure the continuity of health care.
    • Participating in supervisors’ meetings to facilitate the exchange of information and “inter-phase” collaborations.
    • Ensure respectful relationship and collaboration with MOH, communication with the care givers/patients and all the team members.
    • Carry out other relevant/delegated duties as may be required, or assigned by his/her supervisor etc.

    Data Entry Operator - Malaria / Nutrition Peak - Bauchi

    Job Description

    • Enter medical data (e.g., patient data, dates of visit, content of prescription, viral load, etc.) in the mission’s database on a regular basis and according to MSF protocols.
    • Participate in the compilation of data both from MSF structures and from collaborating health facilities.
    • Participate in the search for missing data.
    • Update files of existing patients and make any corrections needed.
    • Make regular back-ups of all data processed.
    • Look after all equipment provided and keep the recording area clean and tidy, giving special importance to all patient files.
