    Medical Doctor (Gummi)

    Main Objective and Responsibility of the Position

    • Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions.

    Accountabilities

    • Apply medical knowledge and skills to diagnose and treat patients. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols. Engage in prevention activities.
    • Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the patient treatment and follow-ups after discharge.
    • Follow the evolution of the hospitalized patients, through daily visits, consultations and examinations, prescribing the necessary treatment according to MSF protocols, deciding whether they can be discharged or transferred to other departments (in collaboration with other doctors and informing their family about the patient’s evolution).
    • Monitor the rational use of medicines and equipment under his/her responsibility and take care of the quality, disinfection and sterilization of the medical material. Ensure general compliance with standing hygiene standards.
    • Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patient’s illness, medical error and monitor the proper functioning of the department, equipment or material.
    • Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care.
    • Ensure universal precautions are followed at all times, reducing biohazard risks and improving infection control.
    • Ensure professional confidentiality is respected.
    • Manage the team under his/her direct responsibility according to MSF HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.

    MSF Section/Context-Specific Accountabilities

    • Conduct medical visits/medical consultations in the Triage, HDU, ITFC (transition, phase 1 & 2), IPD and isolation areas.
    • Participate in medical meetings, ward rounds and all Mortality reviews.
    • Regularly analyze patient files for completeness and check if treatments prescribed have been followed
    • Correctly fill in forms such as laboratory request, blood transfusion, treatment form, etc.
    • Always give feedback to the requester and the progress of the work to the line manager.
    • Communicate the information to the appropriate person.
    • Plan his/her weekly activities and submit to the supervisor.
    • Adhere to defined tasks as outlined in weekly/monthly activities.
    • Apply knowledge and skills to diagnose and treat patients.
    • Objectively manage specific treatments: oxygen therapy, adrenaline treatments, use of the nebulizer, and react to their fluctuations.
    • Apply knowledge and skills to diagnose and treat patients during consultations and ward rounds respecting MSF protocols.
    • Conduct follow-up visits and adjust management according to the protocols and the patient's current clinical conditions or the results of laboratory examinations.
    • Identify patients in need of referral and discuss with the line supervisor, ensure the referral papers are ready and completely filled before departure, ambulance contacted, and database updated as per the referral policy.
    • Keep a close eye on what other members of the medical team are doing.
    • Apply objectively knowledge and skills in the management of patients.
    • Discuss with colleagues the challenges/errors discovered regarding the activities in each care unit (Triage, HDU, ITFC, General IPD & Isolation).
    • Contribute, through his/her behavior to maintaining a calm working atmosphere and teamwork.
    • Identify challenges related to daily activities, report and discuss ways forward with the line manager.
    • Provide clear explanations of the illness and initial psychological support to patients and/or caregivers in need.
    • Report medical incidents and participate in their management.
    • Establish a link with the psychosocial team.
    • Report ASAP all COC & PSA cases using the right, confidential and secure channel

    Requirements (Qualifications / Experience / Knowledge)

    Education:

    • Registered Medical Doctor Diploma Desirable diploma in Tropical Medicine, Paediatric & Nutrition/SAM associated with complication.

    Experience:

    • 2 years’ experience minimum as a Medical Doctor or in clinical work. Desirable in tropical medicine, Public Health, paediatrics, infectious diseases, general practice.

    Languages:

    • Mission language essential. Local language desirable.

    Knowledge:

    • Essential computer literacy (word, excel)

    Medical Doctor (Talata Mafara)

    • The Medical Doctor under MTL leadership is overall responsible for medical care provided to all patients accessing the pediatric ward of Anka General Hospital from the screening tent to the ward through triage and Emergency room and OPD locations(IDP Camp and ATFC).
    • He is leading patient care in close collaboration with Nurses , Clinical Assistants, Hygiene agents, lab technicians… to ensure patient safety.

    General:

    • Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
    • Informs patient and/or his/her family about the illness, treatment provided, the evolution and provide appropriate explanations about the treatment to follow, checking if they understood.
    • Follow up the evolution of the hospitalized patients, through daily visits and decide to discharge or transfer to other departments/facilities – in collaboration with other doctors.
    • Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care.
    • Knows and ensures all MSF medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control.
    • Ensures professional confidentiality is respected.
    • Ensure absolute confidentiality of medical data and patients’ information.
    • Responsible for patient safety and apply patient centred care
    • Correct use of MSF properties.
    • Carry out duty in the assigned ward by the line manager
    • Present at the duty station in due time (According of the roster).

    Patient care Management:

    • Knows, understands and complies with admission and discharged criteria available in the ER and strict adherence to it.
    • Strict adherence to admission process, this includes the correct use of the admission file (pink for non-SAM and Yellow for SAM patients) respecting all steps of patient examination and initial Management.
    • Writes clearly complains and take meaningful history of the current sickness, Physical examination, Lab investigation, initial diagnosis and treatment plan.
    • Apply actively ETAT principle to identify and treat urgently patients at higher risk and ask for support whenever needed.
    • Carry out daily ward round in the assigned ward, update diagnosis according to new findings and adapt the treatment plan. Deep and detailed patient examination after thorough review of documentation from the previous colleague.
    • Discharge patients who meet the discharge criteria
    • Proactively Identify patients who need referral to another referral hospital, prepare the referral sheet, inform the caretaker then your line manager to organize the movement.

    Drug Management:

    • Correct use of the prescription for all oral drugs at discharge including bednet for all new admission
    • Participate in the Monthly consumption Analysis
    • Rational use of Antibiotics

    Data Collection and Reporting:

    • Fills the epi front page for all exits
    • Fills the inpatient Mortality sheet immediately after patient death.
    • Actively prepare and participate in the Mortality review case.
    • Writes Medical Incident Report whenever it occurs and report to the line manager

    Medical Leadership:

    • The Medical Doctor is leading the overall patient care, he should therefore be a very good communicator and a team player, guided by Medical Ethic, MSF principle and Values, respectful and respecting all (Para)Medical colleagues.     Requirements (Qualifications / Experience / Knowledge)
